Care Coordinators help patients to navigate and manage their way through a complex healthcare system often while facing the most daunting challenge of their lives. Coordinators do not give medical advice but their role is to educate, support and empower the patient through and after a medical crisis. Care Coordinators are members of the healthcare team who help facilitate good communication between the patient and the medical team, identify the barriers to healthcare for the patient and connect them with the resources they may need, such as financial assistance, counseling, or transportation.
